Abstract

Pre-school upbringing of children with developmental difficulties is the first link in the system of education and upbrining and at the same time the utmost segment in the system of integrational rehabilitation. Initiating the process of pre-school upbringing and adequate organized treatment at early age, permits support of the development and maximum usage of the remained capabilities of children with developmental difficulties. The newest world experiences show that the upbringing and educational integration of children with developmental difficulties as asegment of the social integration, should set the main conditions in the frames of upbringing education and rehabilitational continuity and support the minimal restrictive environment for their development. In the pre-school institutions that function on the territory of the Republic of Macedonia, an indifferent attitude towards the children with developmental difficulties is present, due to the uninformed educators for the characteristics and capabilities of these children and their believes for the negative influence of the children with developmental difficulties on the children without impairments, that implicates involvement of subsidery defectological education of present employed staff in the mainstream pre-school institutions, also including defectologists in kindergartens for work with children with special difficulties in their development. Also, the data about the educator’s attitude in the mainstream preschool institutions is disturbing, so that they put the accent on the obligation for reducing of the program for work with handicapped children, not taking into consideration that subsidery, special forms and methods are needed beside the programes that are for the main stream group. The goal of our paper is to point out the need of involving of children with light developmental difficulties; the existence of special team in the pre-school institutions when involving children with developmental difficulties in integrational conditions, will follow the child for a longer period and will enable interdisciplinary approach in diagnostics, work and evaluation of the capabilities of the child, like a base in planning of the upbringing and educational work with children at pre-school age.

EARLY DETECTION, DIAGNOSIS, TREATMENT AND PRE-SCHOOL EDUCATION OF CHILDREN WITH SIGHT DAMAGE

Abstract

Sight damages can appear in every period of the human life, from birth till very old age. The disorders of the sight function retard and hinder the possibility of learning, proper informing as well as recognizing the objects in reality. The possibility for rehabilitation and adaptation is bigger and the psychical consequences in the person development are smaller, if the person with damaged sight is younger and rehabilitation and correction of the incorrect sight function start at proper time, i.e., immediately after the detection of the damage. The developmental deviation with children with damaged sight can be recognized in the general physical development, in poorer fund of real ideas in the sensomotor development, in the space notion and space relations, in reduced motivation for certain activities and so on. However, these children have the same needs as the children with normal sight, such as: love, sympathies, care, discipline, learning, understanding, patience and so on. It should be taken into consideration that each child is an individual which means a special approach to the rehabilitation. In fact, more often we find children who, besides the sight damages, have other damages such as: cerebral paralysis, hearing damages, mental retardation, emotional disorders and so on. The preventive measures contribute to the decreasing number of children with more difficult psycho-physical anomalies and they mean obligation of the community to organize: Advisory Institutions for pregnant women, quick and prompt intervention of the ophthalmologists and other experts, increasing of the general culture of the population and so on, and the systematic sight check-ups of every child should be performed in the first, third and sixth year of their lives. The expert treatment of sight handicapped children should start from the very detection of the sight damage (blindness and low vision). For small blind and low vision children the Developmental Advisory Centers should be organized as well as the expert help for the family of such a child. In that way, the children acquire adequate experience and conditions for their correct psycho-physical development are made. That means we should provide conditions for getting the world closer to the child: with words, motions, feelings, explanation ... The blind child uses his/her hearing, the sense of smelling, his/her hands and the hands of the others to acquire necessary knowledge of the environment. The children with damaged sight at pre-school age should be placed in younger, middle and older educational groups in order to provide correct and educational work, specially adapted to each group of handicapped children. In such a way, the children with damaged sight can get ready for starting the adequate school, according to the sight and the level of the sight damage, intellectual and other abilities.

Abstract

The problems of mentally retarded persons which we encounter in our everyday work and several examples with drastically negative consequences of not planning the family have induced us to report our view points and suggestions about the necessity and importance of early detection of disability and necessary actions to help mentally retarded persons especially females in family planning and protection from sexual abuse. The negative consequences are reported by illustration of one case of mentally retarded mother with seven mentally retarded children and herself born illegitimately also by mentally retarded mother.

Abstract

The term disharmonious is determined against the harmonious development of children. It is stressed that these children, because of the whole bio-psycho-social growth which is at a lower level, very often react with psychical disorders, compared with the children which normally develop and this seems to be not only a serious problem in the diagnostics but also in the performing of the adequate and in due time treatment. Trained by the existing experiences in the work with these children it is indicated that the best differential-diagnostics is performed in conditions of Day hospital where a whole treatment is provided by a professional team. In the treatment in this category of children it is insisted on the respect of the developmental dynamic approach, and for this, the acknowledgments that are given by the neuropsychology are used. The need of early treatment of a child, parents and the closest environment is stressed because if these children are neglected, they show the tendency to organize themselves at their lowest values, they intellectually decline and lead to severe psycho social results .Working with these children in our Day care hospital although we use a great number of therapeutic activities, we put the stress on the application of psychomotor re education. The use of remedial therapy as the only technic, without including the parents in the working plan, has shown as unadequate. Although there is an intellectual insufficiency, we tend these children to be mentally healthy, if we succeed to achieve bigger harmony in the function of the cognitive, emotional, motor and social aspects of the individual. Providing continuity in the treatment of this category of children, it is insisted to provide the pre-school institutions with planned and professional team work. The stress is put on the defectological activity, in a way to prepare the children to enroll in school.

Abstract

Inflammation processes of the middle eye cover, take significant place in the ophthalmology. This is confirmed by the information of several authors. Representation of these inflammation processes in children to 15 years of age according to Smith R. 1983, is 1-10% . This disease is too often chronic and redundant and the inefficacy of the therapeutic approach, i.e. the appearance of the refraction, manifest in the anatomical and functional disorders of the organ of sight. Taking this into consideration in the presented material, it’s specially stresses the complexity of the problem of ophthalmia that comes out from the large number of ethno-patho-genetic factors and the connection of the numberless systematic mechanisms.

Abstract

Factors which contribute the appearance of handicaps are numerous and complex. By their nature they are inherited or determined by factors of outer environment, mostly as a result of interaction of both. In the broad sense the genetic factors are located in 46 chromosomes in the human cell, before all in the female and male fertilized cell e.i. the genes located in the chromosomes which determine the function of the central nervous system. The chromosome aberrations e.i. the visible aberrations of the chromosome structure presents the cause for the appearance of handicaps mostly together with some syndrome. That’s the case with Dawn’s syndrome( mongolism) where we can find trizomia of 21 chromosome. Relatively, the great number of examinees (176) which we discovered in a period of 10 years from all the places of R. Macedonia confirm the need for further investigations. The newborns with this syndrome are children with prenatal development finished in not adequate way. The presence of different physical appearance on the birth and later on, more or less, speaks about indefinite mixture of pathological factors in the growth of the child and the difference. The symptoms that are discovered in these children indicate that they are retarded in their pchylological and physical development from the normal children, and the further growth and development are on a lower level. The psychological, pediatrical and psychiatrical researches show that the level of intelligence of our examinees is low, where as the social adjustment is far more better. The examination of our examinees was made up of taking an anamneza from the parents, mostly from the mother. Every examinee was under taken an caryological test. Ten of them showed an translocative type of Dauwn’s syndrome where one of the parents is the carrier of the heterological balanced translocation. The heterological balanced translocations can be discovered only through affected newborn child. Our researches show that the age level of mothers of the mongoloid children does not differ from an average in relation to mothers in the normal population. Because the declining age of a mother can be the reason for changes in oogeneses, the average of mothers` declining age is far smaller. Through our research we found a great number of mothers who had given birth to a mongoloid child in the period when they were physiologically most capable for fertilization.
