ANESTHESIA IN PATIENTS WITH HEAD AND NECK MUCORMYCOSIS

Abstract

Abstract Aim: Mucormycosis is an opportunistic fungal infection, especially seen in immunosuppressed or diabetic patients. The majority of patients are hemodynamically unstable and may have multiple organ failure in this fungal infection of which treatment is antifungal therapy and surgical debridement In this review, we aimed to point to the difficulties associated with anesthesia in patients who are scheduled for surgical intervention with the diagnosis of mucormycosis. Discussion: Patients with mucormycosis have a mortality rate of up to 95% due to their high level of comorbidities. Sepsis, multi-organ failure and poor hematological profile (e.g. pancytopenia) are frequently associated with these patients. Preoperative evaluation is as important as anesthesia practice in these patients. Intravenous access and airway management may be difficult and patients may require major blood transfusion and hemodynamic support. However, some patients have diabetes mellitus and even diabetic ketoacidosis. Therefore, close monitoring and regulation of blood glucose is required in these patients. Multidisciplinary approach is necessary in these patients due to infections, organ failure, blood transfusions and medical antifungal therapy. The location and size of the surgery should also be considered in terms of preoperative preparation. Conclusion: Patients with mucormycosis require attention for anesthesiologists because of their immunosupression and accompanying comorbidities.
