Clinical characteristics of children with cholelithiasis – experience of the two centers

Abstract

Introduction. Cholelithiasis has been diagnosed in children more frequently over last few years. The frequency of cholelithiasis in children is assessed to range from 0.13 to 1.9%.Aim. The aim of the study was to evaluate risk factors, course of cholelithiasis and its complications in patients selected from two pediatric liver centers.Material and methods. 113 patients with cholelithiasis treated in two clinical centers between October 2012 and September 2013 were enrolled into the study.Results. The frequency of cholelithiasis was similar in boys and girls (50.4 vs. 49.6%). The median age of onset of cholelithiasis was 5 years. In 15 patients (13.3%), other chronic diseases were reported. Other risk factors were described in 39.8% of patients. The most common risk factors were: family history of cholelithiasis (32.7%), recurrent urinary tract infection (15%) and obesity (11%). Asymptomatic course of disease was observed in 39 patients (34.5%). The most common symptom of cholelithiasis was paroxysmal, abdominal pain with typical location which was reported by nearly 40% of the patients. Surgery or ERCP was performed in 28 patients (25.2%), whereas 68 patients (60.7%) were treated with pharmacotherapy. In 25 patients (22.3%), no therapy was applied. Selected patients were treated with surgery and pharmacotherapy at the same time (therefore, the sum of percentages exceeds 100%). Complications occurred in 15 patients (13.4%) and the most frequent one was acute pancreatitis.Conclusions. Family history of cholelithiasis was the most frequent risk factor observed in the analyzed sample of patients. Taking into account the above and lack of other significant risk factors identified, we conclude that further genetic investigation should be performed to explain pathogenesis of cholelithiasis in children.
