Gingival recession management in orthodontic treatment – a case report

Abstract

Gingival recession (GR) is one of the most common and undesirable defects, affecting from 60% (younger patients <20 years old) to 90% (older patients >50 years) of the population in the Western Europe (18). It is more frequent in mandibular than maxillary teeth, and more frequent on labial than lingual surfaces (7). Can orthodontic treatment help manage gingival recession, or to the contrary, is the risk of GR formation increased due to previous orthodontic treatment? Is the problem of gingival recession manageable at all during therapy with fixed appliances, or is it so unpredictable that it can improve only by chance? <b>Aim.</b> The aim of the article is to present a case of a female patient presenting with increased gingival recession on both upper lateral incisors, and to describe a management procedure applied in order to improve the gum condition. <b>Material and methods.</b> Treatment of a 27-year-old female presented with severe gingival recession on both upper lateral incisors was described. Passive self-ligation appliances were used in treatment (Damon System). The treatment plan was to correct the position of these teeth to the middle of the alveolar crest in order to create better conditions for gingival recession improvement. <b>Results.</b> Thanks to carefully delivered and planned treatment gingival recession improved after treatment lasting a little over 2 years. <b>Conclusions.</b> Diagnostics, planning and introduction of treatment aimed to position the roots on the middle of the alveolar crest may, under favourable conditions, help improve condition of the marginal gingiva and gingival recession. Orthodontic treatment managed in a professional and responsible way may be utilized to tackle the problem of gingival recession. <b>(Sanocki G.
