Hypo-hyperdontia – a review of the literature

Abstract

Hypo-hyperdontia (or concomitant hypo-hyperdontia, CHH) is a very rare anomaly when congenital agenesis of tooth buds and presence of supernumerary teeth occur in the same patient at the same time. Its prevalence rate is estimated to range from 0.002% to 3.1% of the general population and it is found more frequently in permanent dentition, in males and it is usually concomitant with other genetic disturbances. The exact etiology of concomitant hypo-hyperdontia (CHH) has not been discovered yet. It involves both genetic and environmental factors which cause dental lamina proliferation disturbances during the process of ontogenesis. <b>Aim</b>. The aim of this paper was to review the literature and to highlight the CHH using as an example the case of a patient treated by the Department of Orthodontics of the Medical University of Warsaw. <b>Material and methods</b>. Related literature published in the years 1967-2017 was reviewed using the PubMed base and the following key words: hypo-hyperdontia, hypodontia, and hyperdontia. Studies by Polish authors describing the issue under discussion were also included in the review. A medical interview and physical and radiological examinations of an 8-year-old patient were conducted.<b>Results</b>. Following a clinical examination and some additional ones, an isolated maxillarymandibular form of hypo-hyperdontia was diagnosed in the patient and a preliminary therapeutic management plan was worked out. <b>Conclusions</b>. Concomitant occurrence of congenital agenesis of tooth buds and the presence of supernumerary teeth occurring in the same patient is a very rare anomaly related to the number of teeth. In the Polish literature on the subject, this paper represents one of the few descriptions of a patient diagnosed with CHH. In a combined orthodontic, surgical and prosthetic treatment of patients with hypohyperdontia, besides standard therapeutic management methods (observation, extraction of supernumerary teeth, gap closing or reconstruction of socket sites for the missing teeth), the procedure of autotransplantation should be considered. <b>(Czerkies M, Jurek A, Podobas-Muderrisoglu B, Drohomyretska M, Zadurska M.
