Mesiodens – incidence, complications and methods of early orthodontic and surgical treatment procedures based on material supplied by the Department of Orthodontics of the Medical University of Warsaw

Abstract
Introduction. A mesiodens is a supernumerary tooth locatedin the mesial nine between the central incisors in the maxilla,and less frequently in the mandible. Its prevalence, mainly inpermanent dentition is estimated at 0.15-4%. It is usuallyconically shaped, with normal histology, sometimes it resembles a typical incisor. Aim. The aim of the study was (1) to definethe incidence of the occurrence of maxillary mesiodentes basedon the documentation of patients being treated in the Departmentof Orthodontics at the Medical University of Warsaw; (2) Analysisof clinical complications related to the presence of a mesiodens;(3) A close examination of methods of orthodontic treatmentand surgical management applied. Material and methods. Thestudy analyzed 2921 of patients’ (1684 girls’ and 1237 boys’)medical health record cards in order to detect the presence ofmaxillary and mandibular mesiodentes. The number ofmesiodentes and the occurrence and type of impacted teethwere assessed, the plans and methods of orthodontic treatmentand surgical procedure used were analyzed, as well as thepatients’ age during their qualification for orthodontic treatmentand mesiodens extraction. Results. After having analyzed 2921health record cards, the presence of a mesiodens was detectedin 53 of them (1.81%) - 19 girls and 34 boys. Most frequently,a single supernumerary tooth was identified. In two thirds ofthe cases impacted teeth were found – usually central incisors.The average age of the children when they first presented fororthodontic examination was approximately 8 years 10 months,and 9 years 2 months at the time of extraction. The most commontreatment method was with a removable appliance (an upperSchwarz plate) introduced after prior extraction ofa supernumerary tooth. Conclusions. Mesiodentes were foundin approximately 1.81% of the patients, more frequently in boys.The presence of a supernumerary tooth caused retention ofupper central incisors. Among other complications, diastema,crowding, rotation, and root dilaceration occurred. In most cases,interdisciplinary orthodontic and surgical treatment wasnecessary.
