Prospective study of clinical profile of Fournier’s gangrene

Abstract

Background: Fournier’s Gangrene is a surgical emergency, a nightmare to surgeon and surgical staffs. The sudden and spectacular swelling of the scrotum followed by its sloughing and the baring of the testicles are dramatic events and after that lead to death. Keeping in mind the fulminating nature of the disease, the surgeon has used a different combination of antibiotics, surgical procedures, dressings and reconstruction procedures. Objectives: To study the clinical profile, evaluate the causative factors, various treatment options and prognosis of patients of Fournier's gangrene. Material and methods: A prospective observational Study conducted in Dept of surgery in our medical college which is tertiary care centre and Government Medical College, Solapur. Duration of study was from Sep 2016 to Aug 2018. 50 patients of Fournier's gangrene were studied. Results: Most common symptom was pain in 45 i.e. 90%, causative factor was idiopathic in 17 i.e. 34%, all patients underwent debridement followed by reconstructive surgery in 22 patients i.e. 44% and mortality was found in 13 i.e. 26% patients of Fournier's gangrene. Conclusion: The mean age of presentation among dead patients is found to be 59 years which is significantly higher than the mean age among survivors which is 54.67 years. The male to female ratio is 48: 2. The source of the disease is most commonly idiopathic, multiple wound debridement are required. The average number of wound debridement required is 1.81.
