Triple assessment for the diagnosis of breast lumps: Combined clinical examination, ultrasonography and fine-needle aspiration cytology

Abstract

Background: The most common presentation of cancer of breast is lump in the breast. Triple assessment of breast, consisting of clinical examination of breast, radiological examination of breast and fine needle aspiration cytology (FNAC) is comparatively simple, reliable, reproducible and cost-effective. Aim: To determine the sensitivity, specificity, positive and negative predictive values of triple test (TT) consisting of physical examination (PE), sonomammography, fine needle aspiration cytology (FNAC) in the evaluation and characterization of palpable breast lump. Material and Methods: was conducted on 50 patients presenting with complaint of breast lump. All the patients were evaluated according to the components of the triple assessment and followed through with histopathological examination of the excised mass. Results: Malignant lesions were found more commonly (62.0%) than benign lesion. The accuracy, sensitivity, specificity, PPV, NPV of triple assessment of breast 
were 94.0%, 100%, 84.2%, 91.2% and 100% respectively with p-value 0.001. Conclusion: Triple assessment of breast is a very useful tool in evaluating the breast diseases. In patients with definite lump, clinical examination (by experienced hands), coupled with cytologic and radiologic help, is sufficient to rule out malignancy.
