A Prospective study to evaluate the functional outcome of Arthroscopically assisted single bundle anterior cruciate ligament reconstruction by Hamstring tendon graft

Abstract
Introduction: Aim of this prospective study was to evaluate the functional outcome of Arthroscopically assisted single bundle anterior cruciate ligament reconstruction by Hamstring tendon graft. Material & method: This prospective study was conducted in the department of orthopaedics of SRMS-IMS,Bareilly, in the period between November 2015 – July 2017 after obtaining approval from hospital ethics committee. Total of 20 patients of >18 years age presenting with unilateral knee complaints and history of trauma to the knee in theorthopaedics emergency and outpatient department of SRMSIMS, Bareilly were evaluated by a thorough general and local clinical examination of the knee. They were treated by arthroscopically assisted single bundle anterior cruciate ligament reconstruction with Hamstring tendon graft. Postoperatively at each follow up at 3 weeks, 6 weeks, 3 months, 6 months results were assessed using range of motion, Lachman testing, Pivot shift testing and at final follow up using IKDC, Lysholm Knee Scoring Scale functional outcome was evaluated. Result: Good results was achieved regards to functional outcome. At final follow up all patients achieve full range of motion, 16 cases(80%) had negative lachman and 4 cases(20%) have grade 1, one case (5%) had developed +1 pivot shift while no shift or normal grade was present in 19 cases (95%). There was significant improvement in IKDC score and Lysholm score. Using lysholm grading 18 cases(90%) shows good results 1 case (5%) shows excellent and 1 case(5%) fair result. Conclusion: Functional outcome remain good in this study.Single bundle ACL reconstruction with four string hamstring graft by transportal femoral tunnel placement technique allows for anatomical tunnel placement and improved knee biomechanics. Post –operative complications are also less with this technique
