A retrospective study of primary repair or diversion colostomy in the patients with injury to colon at tertiary health care centre

Abstract
Background: Gastrointestinal perforation, also known as ruptured bowel, is a hole in the wall of part of the gastrointestinal tract. Aims and Objectives: To Study Primary Repair or Diversion Colostomy in the patients with injury to colon at tertiary health care centre. Methodology: This was a cross-sectional study carried out in the patients with injury to colon in the department of General surgery at tertiary health care centre during the one year period i.e. March 2017 to March 2018. In the one year period with written and explained consent there were 50 patients with intestinal perforation were included into the study. The statistical analysis was done by unpaired t-test, chi-square test and Z-test (Standard error of difference between two proportions) and was calculated by SPSS 19 version software. Result: Age in Group A was 39±3.42 yrs. and 40±3.12 yrs. in Group B was comparable (p>0.05; t=1.56; df=68). The sex ratio in Group A was 2.12 and 1.77 was comparable (p>0.05, X2=0.92, df=1). Wound infection were 12% and 20%; Intraabdominal abscess-4% and 16%; wound dehiscence-4% and 16%; Septicemia-4% and 12%; Anastomotic leak and fistula formation- 0% and 8%; Respiratory complications- 4% and 8%; Prolonged ileus- 4% and 8% respectively in Group A and Group B. Overall complications were significantly higher in Group B i.e. 32% as compared to 16% in Group A (Z=2.26, p<0.05) Conclusion: It can be concluded from our study that in the patients with colon injury primary repair was superior as compared to diversion surgeries with respect to less Wound infection, Intraabdominal abscess, wound dehiscence, Septicemia, Anastomotic leak, Respiratory complications.
