A study on retinal changes in the macular area in Retinitis Pigmentosa (RP) with spectral domain optical coherence tomography (SDOCT) in patients attending a tertiary care hospital in central India

Abstract

Background: Retinitis Pigmentosa is a progressive degenerative disorder of retina affecting rod and cone photoreceptors and retinal pigment epithelium with reduced visual function. This study was done to study and quantify the change in retina in retinitis Pigmentosa patients using optical coherence tomography technique. Methods: The current prospective hospital based case series study was conducted in a tertiary care Eye Hospital of Nagpur city (Maharashtra) during study period October 2014 to October 2014. Ethical consideration were met through institutional ethical committee. Informed consent was obtained from all subjects before the procedures. The diagnosis of Retinitis Pigmentosa was based on the clinical findings including compromised night and side vision, characteristic findings on 78 diopter stereoscopic evaluation and indirect ophthalmoscopic examination, including peripheral retinal pigmentary changes with arteriolar narrowing and a pale optic disc. Results: In retinal layer analysis, it was found that most common layer to be absent at fovea was the interdigitation zone (67%) followed by the IS/OS junction (31.4%) and the external limiting membrane (31.4%). The most common layer to show clinical thinning at fovea was outer nuclear layer 34.3%) followed by the retinal pigment epithelium (22.5 %). The mean central subfield thickness was found 236 ± 63 microns. The retina was thickest in nasal outer macular subfield (283.549 microns) and thinnest in outer temporal macular subfield (226.30 microns). Conclusion: It was concluded that Retinitis pigmentosa predominantly affecting the outer retinal layers especially the photoreceptors and associated with significant retinal thinning. OCT is a valuable tool in the detection of macular changes in Retinitis pigmentosa cases.
