Acetabular fractures- outcome analysis

Abstract
Introduction: Fracture of acetabulum poses a significant management issues for orthopedic surgeons. These are usually caused by high-energy trauma and are associated with other major injuries. The fracture or fracture dislocation produced depends on the magnitude and the direction of the injuring force as well as strength of bone. Materials and methods: Fifteen patients from December 2015 to December 2016 presenting with acetabular injuries to the emergency services of our hospital were included in the study. They were managed either surgically or conservatively based upon individual fracture pattern and patient characteristics. Follow-up was done to evaluate the outcome. Results: There were thirteen males and two females. Mean age of presentation was 42.6 years. Surgery was done in 8 cases – ORIF with screws and reconstruction plates; 7 cases were managed conservatively – closed reduction of posterior hip dislocation and supracondylar/ trochanteric pin traction. Reduction achieved was deemed ‘good’ in 7 cases and ‘satisfactory’ in 8 cases. Conclusion: The goal of treatment is anatomical restoration of articular surface and joint stability; however this does not make surgical intervention absolutely necessary and every acetabular fracture doesn’t need open reduction and stabilization. Management should be tailor-made to every patient for best possible outcomes. In the present study individualized treatment for each patient was provided to achieve best possible outcome for each case. Long-term outcome however needs to be assessed for hip arthritis
