Addressing missed opportunity in immunization among underfive children in a rural tertiary care hospital of Darjeeling district, West Bengal, India

Abstract

Context: Immunization services are the most cost effective public health interventions in preventing vaccine preventable diseases. However, missed opportunity in immunization and vaccine hesitancy exists.Appropriate administrative support ensures effective utilization of these services. Aims:Determining prevalence of missed opportunity in immunization among under five children attending North Bengal Medical College and Hospital (NBMCH) and ascertaining reasons for vaccine hesitancy. Developing managerial strategies to address the concerning issue. Settings and Design:Descriptive, cross-sectional hospital based study was carried out for two months in paediatrics outdoor of NBMCH with a sample size of 100, selected by systematic random sampling technique. Methods and Material:Those eligible for vaccination on that day were included in study. Exit interview was conducted on respondents (caregivers accompanying study subjects) with help of predesigned pretested semistructured questionnaire on outdoor days. Health staffs engaged in immunizations were enquired.Statistical analysis used: Descriptive statistics in form of percentage and chi square test (inferential statistics) Results:35% children though eligible for vaccinations, did not receive complete course. Major factors elicited for vaccine hesitancy were lack of clarity in advice given, vaccine unavailability, cultural perceptions, taboos and not administering all the required vaccines simultaneously by health staffs. Literacy status of respondent was significantly associated. Managerial grid was developed and reasons were categorized, prioritized and plausible solutions formulated.Conclusions:Missed opportunity in immunization exists and the factors of vaccine hesitancy elicited needs to be addressed at all levels through sensitive interventions. Managerial remedies are expected to increase service utilization. A larger study is recommended.
