ALT SERV‹KAL SP‹NAL CERRAH‹DE KOMPL‹KASYONLAR

Abstract
Servikal spinal bölgenin ameliyatlar› çeßit olarak çok say›da ve sonuçlar› bak›m›ndan a¤›r komplikasyonlara neden olabilir. Her operasyon gibi alt servikal spinal bölge (C3-C7) girißimleri üç tür komplikasyon riski taß›r: I. Sistemik duruma ve anesteziye ait komplikasyonlar, II. Pozisyona ait komplikasyonlar, III. Direkt cerrahiye ba¤l› olarak gelißen komplikasyonlar. Bu yaz›da direkt cerrahiye ba¤l› olarak gelißen komplikasyonlar gözden geçirilmißtir. Bu komplikasyonlar ßu alt› gruba ayr›lm›ßt›r: 1. Komßu dokularda yaralanma (karotis arteri, jugular ve brakiosefalik ven, vertebral arter, rekürren laringeal sinir, hipoglossal sinir, sempatik zincir, duktus torasikus, trakea, özofagus, plevra, dura, omurilik, servikal kökler), 2. Enstrüman ve/veya greft sorunlar› (enstrüman malpozisyonu, greft dislokasyonu, vertebran›n avasküler nekrozu), 3. Postoperatuvar hematom, 4. Enfeksiyon, 5. Greft donör yeri sorunlar› (hematom, enfeksiyon, kozmetik problemler ve greft donör bölgesine has sorunlar), 6. Geç komplikasyonlar (enstrüman yetmezli¤i, enstrü- mantasyona ya da grefte ba¤l› geç organ yaralanmalar›, greft psödoartrozu ve çökmesi, deformite gelißimi, füzyonun alt ve üst uçlar›ndaki seviyelerde dejenerasyon). Her bir komplikasyon s›kl›k, nedenleri, korunma ve tedavileri aç›s›ndan tart›ß›lm›ßt›r.
