Anaesthesia for a patient with Co-arctation of aorta undergoing cesarean section
Journal Title: Medpulse International Journal of Anesthesiology - Year 2017, Vol 3, Issue 2
Abstract
Pregnancy and its associated challenges in cardiovascular physiology present a unique challenge in a woman with an underlying heart disease. In this case a 23 year old primi-granida, 38 weeks of gestational age, with coarctation of aorta was posted for elective caesarean section. 2D-Echo showed EF=62%, concentric LVH, bicuspid aortic valve, coarctation of aorta distal to left, mild AS, mild AR. During the procedure, intraoperatively hemodynamics were monitored and B.P remained stable throughout (111/70 to 130/82 mm Hg). At the end of surgery the patient reversed and extubated successfully. New-born APGAR SCORE was of 8 and 10 at 1 and 5 min respectively. Coarctation of aorta in a pregnant patient is a unique challenge to the obstreticians and anaesthesiologists and requires a thorough understanding of the impact of pregnancy on hemodynamic response to coarctation of aorta. Purpose of this paper is to provide an overview of the preoperative and intraoperative implications of CHD for the anaesthesiologist involved in the care of adults with CHD undergoing non cardiac surgery.

Comparison of post-operative analgesic efficacy of bupivacaine and ropivacaine using ultrasound guided oblique subcostal transverse abdominis plane block in open cholecystectomy under general anaesthesia
Background: Cholecystectomy involves upper abdominal incision with considerable postoperative pain. Adequate postoperative analgesia has to be provided to prevent major pulmonary complications in the postoperative period...
Comparative study of haemodynamic response of propofol with ketamine versus thiopental sodium as sole anaesthetic agent in minor surgical procedures
Background: Day care surgery and anesthesia is a upcoming specialty and need of hour in today's era. Most of the minor surgery are done in day care, for whichearly recovery from anesthesia and surgeryis a primary goal of...
Role of oral clonidine premedication for decreasing intra-operative bleeding with respect to scoring system at tertiary health care centre
Background: Functional Endoscopic Sinus Surgery (FESS) is gaining popularity over past two decades as it is helpful in restoring the drainage and aeration of paranasal sinuses, while maintaining the natural mucociliary c...
Effect of carbon dioxide pneumoperitoneum on acid base balance during laparoscopic surgeries
Background: Carbon dioxide is the most commonly used gas to induce pneumoperitoneum during laparoscopic surgeries, as CO2 levels in blood and expiratory gas can be easily measured and monitored. However, insufflations of...
A randomized, prospective, comparative study to evaluate the sensory blockade properties of 3ml of 0.5% isobaric levobupivacaine and 3ml of 0.5% isobaric ropivacaine for spinal anaesthesia in patients undergoing elective lower limb orthopaedic surgeries
Background: Regional anaesthesia is the preferred technique for most of the lower abdomen and lower limb surgeries. Bupivacaine has increased incidence of fatal cardiac toxicity after accidental intravascular injection,...
