Anaesthetic Management of High Risk Chronic Kidney Disease Patient for Continuous Ambulatory Peritoneal Dialysis Catheter Insertion with Omentectomy
Journal Title: Indian Journal of Anesthesia and Analgesia - Year 2018, Vol 5, Issue 8
Abstract
A patient with Chronic Kidney Disease (CKD) having history of Ischaemic Heart Disease (IHD) and Interstitial Lung Disease (ILD) was planned for CAPD catheter insertion with omentectomy. Usually CKD patients requiring general anaesthesia (GA) or spinal anaesthesia present a number of challenges to anaesthesiologist. This patient carried high risk for general anaesthesia due to interstitial lung disease. We managed this high risk case with bilateral Transversus Abdominis Plane (TAP) block and spinal fentanyl with very low dose bupivacaine hyperbaric 0.5% (0.5 ml) to reduce perioperative morbidity. Anaesthesia technique was adequate for conduct of surgery with minimal supplementation and no complications noted in the perioperative period. It was observed that this technique may be a feasible alternative option especially in patients who carry high risk of morbidity with general anaesthesia/spinal anaesthesia with regular dose of bupivacaine.

A Comparative Study between Dexmedetomidine 1 µg/kg and and 25mg/kg of 50% Magnesium Sulphate as Adjuvants with 0.5% Lidocaine for Intravenous Regional Anaesthesia
Intravenous regional anaesthesia (IVRA) also called as Bier’s block is a commonly used anaesthetic technique for surgical procedures on the upper extremities. Many adjuvants have been used to improve the quality of intra...
Comparison of the Effect of Ketamine, Tramadol, 1.5% Saline and Normal Saline Gargle on Post-Operative Sore Throat after Endotracheal Intubation
Postoperative sore throat (POST) is a frequent complaint following endotracheal intubation with incidence rates varying from 14.4% to 61%. Regardless of the incidence or duration, POST is rated as a patient’s 8th most u...
Effect of Positioning after Induction of Spinal Anaesthesia with 0.5% Hyperbaric Bupivacaine in Orthopedic Procedures
Context: Spinal anesthesia is considered safe over general anesthesia but has some side effects. To overcome its side effects, it can be given in sitting position and after two minutes patient can be kept in supine posit...
Comparative Evaluation of two Different Intravenous Doses of Midazolam to Aid the Insertion of LMA Classic as Adjuvants to Propofol Anaesthesia
Background: Propofol is a widely accepted medication for the uccessful insertion of laryngeal mask airway, but as a sole induction agent, it has a very low success rate. A number of co-induction agents have been used w...
Endotracheal Tube Cuff Pressure Monitoring Prospective Double Blind Randomised Control Study
Background: Endotracheal intubation by William McEwan in1878 remained the foundation stone of modern anesthesia. After successful use of laryngoscope and demonstration of laryngoscopy technique by Chevalier Jackson in 19...
