Antiepileptic drugs: interaction, specificity of application

Abstract

When carrying out the pharmaceutical care of a particular patient (for example, an epileptic patient), the doctor and pharmacist must take into account the possible fact of interaction between anticonvulsants among themselves (with polytherapy) or with preparations from other pharmacotherapeutic groups (in the treatment of concomitant diseases, prevention of adverse reactions, etc.). In addition to interaction, an important factor in the release of antiepileptic drugs (AED) is the specific application of the drugs of this pharmacotherapeutic group. The aim of the study was to create a database of possible interactions of AED with each other and with drugs of other pharmacotherapeutic groups. Pay attention to the most important features of the use of anticonvulsants. Methods of research: bibliographic, system analysis. As a result of the studies of the Unified Clinical Protocol of Medical Care "Epilepsy in Adults", the Pharmaceutical Encyclopedia, the recommendations of the International League against epilepsy we can make a preliminary generalization that drugs of the first line choice and those that are most in demand are: valproic acid and its salts, carbamazepine. We have gathered information on possible interactions of AED with each other and with drugs of other pharmacotherapeutic groups. It was established that the greatest number of interactions of AED occurs with such groups of drugs: the drugs acting on the nervous system (respectively, the interaction of AED with each other) – 237 facts; antimicrobial for systemic use – 78 cases; drugs acting on the cardiovascular system – 65 facts, etc. Most often, the interaction of AED are with each other and with the drugs of other pharmacotherapeutic groups occur for drugs of 1 (basic) generation – phenobarbital, phenytoin, carbamazepine, etc. The obtained data can be used for informational filling of the system of electronic recipes. The information will be used both at the stage of prescribing prescriptions by doctors, and at the stage of giving away drugs from a pharmacy. Comparison of instructions of domestic (generic) and imported (original) drugs showed that there are no significant differences in the sections "interaction of drugs", "peculiarities of application" for carbamazepine. However, in appointing drugs valproic acid, doctor should take into account the biopharmaceutical aspects, which are indicated in the instructions for both drugs.
