Αρτηρίτιδα Takayasu. Διαγνωστική και θεραπευτική προσέγγιση

Abstract
Η αρτηρίτιδα Takayasu αποτελεί μια σπάνια κοκκιωματώδη αγγειίτιδα της αορτής και των κλάδων της και πιθανόν και της πνευμονικής αρτηρίας, που προσβάλλει νεαρής έως μέσης ηλικίας άτομα, κυρίως γυναίκες. Οι περισσότεροι ασθενείς εμφανίζουν συνδυασμό στενωτικών και ανευρυσματικών βλαβών. Η νόσος μπορεί να εκφραστεί, ανάλογα με την εντόπιση, την έκταση και τη σοβαρότητα της αγγειακής προσβολής, με συστηματικές εκδηλώσεις, απουσία ή ελάττωση των αρτηριακών σφύξεων, διαλείπουσα χωλότητα άκρων, νεφραγγειακή υπέρταση, εγκεφαλική δυσλειτουργία, αμφιβληστροειδοπάθεια, συμπτώματα από το γαστρεντερικό (κοιλιακό πόνο, ναυτία, έμετο, διάρροια), καρδιακές διαταραχές (δύσπνοια, καρδιακή ανεπάρκεια, στηθάγχη ή έμφραγμα), καθώς και εκδηλώσεις από τους πνεύμονες (αιμόπτυση, πνευμονική υπέρταση). Χωρίς θεραπεία εμφανίζει βαριά πρόγνωση, που όμως έχει βελτιωθεί σημαντικά με τη χορήγηση κορτικοστεροειδών, σε συνδυασμό πιθανόν με ανοσοκατασταλτικά, καθώς και με την έγκαιρη αγγειοχειρουργική παρέμβαση.
