Atipik Prezentasyonlu Bir Kemiğin Paget Hastalığı Olgusu: Olgu Sunumu

Abstract
Kemiğin Paget hastalığı (KPH) artmış kemik döngüsü ile karakterize olup yaygın bir şekilde iskelet bölgelerini etkileyebilen bir kemik hastalığıdır. KPH tanısı genelde başka hastalıkların araştırılması esnasında yapılan tetkiklerde alkalen fosfataz (ALP) seviyesinin yüksek saptanması ve/veya radyolojik bulgu saptanması sonucu tesadüf eseri konur. Hastaların %10-15’inde ALP düzeyleri normal saptanabilir. Nöroşirurji polikliniğine bel ağrısıyla başvuran, lomber MRG’de spondilolistezis ve L4 vertebrada litik lezyon saptanan 72 yaşında bir erkek hastanın yapılan malignite taramasında herhangi bir odak saptanmadı. ALP düzeyi normal sınırlardaydı. Malignite şüphesiyle opere edilen hastanın patoloji sonucu KPH ile uyumlu saptandı. KPH kemik metastazlarının ayrıcı tansında düşünülmesi gereken bir benign durum olup normal ALP düzeyi ile prezente olabileceği ve patolojik materyalin incelenmesi sonucu tanı alabileceği de akılda tutulmalıdır.
