Bir Çocukta Serebral Glioblastoma Multiforme'nin Spinal Leptomeningeal Metastazı

Abstract
Searbral glioblastoma multiformenin spinal metastatik yayılımı ve klinik bulgular vermesi nadirdir. 11 yaşında erkek hasta baş ağrısı ile başvurdu. Manyetik Rezonans Görüntüleme (MRG)’sinde sol lateral ventrikül oksipital hornunda yerleşmiş 3x4x3 cm boyutlarında kitle saptandı. Kitle total olarak çıkarıldı. Patolojik tanı glioblastoma multiforme olarak değerlendirildi. Kontrol MRG 'de rezidü saptanmadı. Bir ay kranial radyoterapi ve kemoterapi uygulandı. Hasta 5. ayda hasta tekrar ani sırt ağrısı ve yürüme güçlüğü ile bize başvurdu. Nörolojik muayenesinde paraparezi saptandı. Spinal MRG’sinde 7.servikal ve 10. torakal vertebra arasında ve lomber bölgede lobule konturlu, kontrast madde tutulumu gösteren intradural intra medüller yerleşimli metastaz saptandı. Tekrar 1 ay kemoterapi ve radyoterapi verilen hasta 11 aydır (5 ay sorunsuz, 6 ay paraparetik olarak) takip edilmektedir.
