Cavernous Angiomas of The Third Ventricle: A Series of Eight Patients and Review of The Literature

Abstract
Background: Cavernous angiomas are vascular malformations rarely found in the ventricular system. Third ventricular cavernous angiomas (TVCA) are rarer and have only been sporadically reported. Methods: We present a series of eight patients with TVCA who were treated in our institute. The clinical data were analyzed retrospectively. Follow-up was obtained based on clinical observation. We also analyzed the characteristics and clinical courses of thirty-four cases of TVCAs that have been reported in the literature. Results: The series includes three males and five females (range: 21 to 38 years; mean 28 years). The initial presentation consisted of mass effect in seven cases and acute intraventricular hemorrhage in one case. None of the patients reported history of seizures. All patients underwent direct surgical excision. One patient underwent translamina terminalis approach, two patients underwent transcallosal transventricular approach and five patients underwent transcortical transventricular approach. Total resection was achieved in six patients while two underwent subtotal resection. Postoperatively, two patients experienced no complications; two patients had electrolyte disturbances; four patients had short-term memory deficits. All patients received follow-up (mean 49.4 months). The two patients who underwent subtotal resection had recurrence; one patient who achieved total resection suffered hydrocephalus postoperatively at six months and had ventriculoperitoneal shunt placed; the remaining five patients had no recurrences. Conclusions: TVCAs are benign lesions; however, symptoms caused by mass effect are often the initial presentation. Microsurgical total resection is the treatment of choice; however, a high rate of postoperative short-term memory deficits is still associated with this procedure.
