Chronic kidney disease – mineral bone disorders and bone biopsy results in children with chronic kidney disease

Abstract
The term renal osteodystrophy (ROD) is used to define alterations in bone histology associated with chronic kidney disease (CKD). The aim of the study was the evaluation of bone abnormalities in children with different degrees of CKD.We studied 92 children with CKD, aged 11.5 ± 3 years, 23 with stage 3-4 CKD and 69 with stage 5 CKD. In all children bone biopsy and measurements of serum levels of creatinine, albumin, calcium, phosphorus, parathormone, osteocalcin, procollagen 1C terminal propeptide, and alkaline phosphatase were performed. In bone biopsy, the following types of ROD were distinguished: hyperparathyroidism-related pattern (HP), osteomalacia (OM), mixed lesion (Mix), adynamic bone disease (ABD), and normal bone histology (NB).The most common type of ROD was NB (36%), followed by HP (30%) and ABD (22%). The HP was significantly more common in children with CKD 3-4 compared to children with CKD 5 (48% vs. 25%). ABD was significantly more common in children with CKD 5 compared to children with CKD 3-4 (27% vs. 4.5%). The type of renal replacement therapy did not affect the rates of specific ROD patterns.The prevalence of HP in children with CRF suggest late referral to the nephrologist this group of patients.
