Clinical and angiographic risk stratification of patients with acute coronary syndrome without ST elevation

Abstract

The risk assessment of patients with acute coronary syndrome without ST segment elevation (NSTE ACS) is recommended by the European Society of Cardiology (ESC) aimed at selection of the best treatment, including invasive strategy. Among many systems of risk assessment of patients with ACS two are particularly recommended: the TIMI and GRACE risk scores. Both systems are based on clinical data and may be obtained after collecting the interview and physical examination on admission to the hospital. The purpose of this assessment is the classification of the patients before treatment to high, moderate or low risk group, which should have therapeutic implications. Risk scores include clinical and laboratory but not angiographic data.Angiography provides information on the character of coronary lesion and the advance of atherosclerotic changes. They have prognostic significance regarding a success of endovascular procedure and long term survival. It is postulated to elaborate and implement more advanced risk assessment in ACS including angiographic and laboratory parameters.
