Clinical Profile of Childhood Asthma and Evaluation of Clinical Efficacy and Outcome in Patients Using Leukotriene Receptor Antagonist and/or Inhaled Corticosteroids

Abstract
BACKGROUND: Asthma is the most common chronic illness of childhood, affecting approximately 10% of children, characterized by variable airway obstruction, caused by partially or completely reversible hyperresponsiveness to various triggering factors. Worldwide, the prevalence of childhood asthma and hospitalization for it are interesting. The most potent inflammatory may be the cysteinyl leukotrienes, which are produced by the 5-lipoxyenase pathway of the arachidonic acid metabolism. These mediators stimulate the production of airway secretions, cause micro vascular leakage and enhance eosinophilic migration in the airways; thus, leukotrienes are believed to play a major role in mediating bronchoconstriction and inflammatory changes pivotal in the patho-physiology of asthma. Anti-leukotrienes are a new class of anti-inflammatory drugs that may have fewer adverse effects than inhaled corticosteroids. AIMS AND OBJECTIVES: 1To assess the outcome of treatment of these patients with inhaled corticosteroids and/or leukotriene antagonists. 2To determine the comparative efficacy of treatment and compare outcome. MATERIAL AND METHODS: An observational and prospective study about clinical and epidemiological profile of childhood asthma and efficacy of leukotriene modifiers in tertiary care hospital. Questionnaire was prepared and observations were taken accordingly. Data was analysed by using Microsoft Excel 2010, manual analysis was also done for some data. Results were expressed in percentage and were compared with various checklists. RESULTS: Montelukast was found to be a satisfactory adjuvant to steroid. Full control was achieved in 70 % of the patients by adding montelukast to steroid as compared to 30% in which control was achieved by increasing the steroid dose. CONCLUSION: From our study it seems that addition of montelukast as an adjuvant to steroid was found to have a significant correlation with the achievement of control and it was found to play an important role in reducing the steroid dose.
