CLINICAL STUDY ON USE OF PREPERITONEAL MESH FOR REPAIR OF BILATERAL INGUINAL HERNIAS

Abstract
BACKGROUND Surgical repair for inguinal hernia is one of the common and a basic procedures encountered by a surgeon in day to day practice. History of surgical repair of hernia dates back to the history of the surgery itself. Despite being one of the most common procedures performed in general surgery, there is a need for a technique for hernia repair with least recurrence and complications. Stoppa described a technique aimed at eliminating hernias by reinforcing the peritoneal space with a giant prosthesis called the giant prosthetic reinforcement of visceral sac (GPRVS). This article shows management of inguinal hernia by not so often practiced Stoppa’s GPRVS. The aim was to evaluate the operative outcome by Stoppa’s modified open preperitoneal Prolene mesh hernioplasty with minimal suturing instead of sutureless procedure using Dacron as proposed by Stoppa in primary/recurrent/bilateral inguinal hernias and to determine the complications. METHODS This study was done in our Government general Hospital, Kakinada between May 2015 and May 2019. 60 patients were included in the study who were managed by open preperitoneal Prolene mesh hernioplasty which is based on Stoppa’s GPRVS. RESULTS It allows truly tensionless and cosmetic repair. It allows repair through a denovo plane which gives added advantage to this procedure to reinforce the myopectineal orifices with nil recurrence rate. CONCLUSIONS This technique besides being relatively easy to perform, has given good results. We would like to recommend modified Stoppa’s repair based on our experience for bilateral and recurrent inguinal hernias.
