Community rehabilitation after stroke

Abstract
Aim: The aim of this paper is to deliver an overview of community rehabilitation services for stroke patients - their purpose, forms, and potential benefits.Views: Stroke is the main cause of disability in the adult population. Rehabilitation is an integral part of medical care after a stroke from the very beginning. It leads to a reduction in disability and mortality after a stroke. Not all the rehabilitation needs of stroke patients can be addressed during hospital stay. Many of them emerge after the patients return home. Many of them develop later after discharge. There is in addition a risk of functional decline after the cessation of rehabilitation. A stroke affects not only the survivors but their families and carers as well. After discharge home both persons after a stroke and their carers should receive multidisciplinary community rehabilitation and support. There is evidence that this type of intervention is effective in reducing mortality, disability or decline in performance in daily activities. It reduces depression and increases the quality of life and satisfaction with services in both patients and carers. Some form of community support (Early supported discharge) in a selected group of patients helps in reducing the length of hospital stay and the overall expenses of care while improving health outcomes.Conclusions: Community rehabilitation is an important element of organized stroke care. It is necessary to develop community services for stroke patients in Poland based on the lessons learned from our own and foreign experiences.
