COMPARATIVE STUDY OF 0.125% BUPIVACAINE HYDROCHLORIDE VERSUS 0.125% BUPIVACAINE HYDROCHLORIDE WITH 25 µgms FENTANYL CITRATE FOR EPIDURAL ANALGESIA IN LABOUR

Abstract
BACKGROUND Labour pains are said to be very intense for many women. They may adversely affect both mother and foetus. A variety of techniques are available to offer labour analgesia. Of all techniques, continuous epidural analgesia has gained popularity as a safe and effective technique of pain relief in labour. This study was undertaken to compare the effect of 0.125% bupivacaine hydrochloride alone versus 0.125% bupivacaine hydrochloride with 25 µgms fentanyl citrate given epidurally for labour analgesia. MATERIALS AND METHODS A total of 100 parturients were divided into two groups each of 50 randomly. All of them were of the age group between 18 to 32 years and were expected to have normal vaginal deliveries. Group-I parturients received 0.125% bupivacaine hydrochloride alone as a 10 ml initial bolus dose and 5 ml top up doses. Group-II parturients received a combination of 10 ml initial bolus dose of 0.125% bupivacaine hydrochloride with 25 µgms fentanyl and top up doses of 0.125% bupivacaine hydrochloride 5 ml with 25 µgms fentanyl. Pain was assessed by Visual Analogue Score, Verbal Rating Score. Vital parameters were tabulated. RESULTS The duration of analgesia with the loading dose in Group-I ranged between 0.6667 hrs (40 min) and 3.0833 hrs (130 mins) with a mean of 1.4333 hrs (86 min) whereas in Group-II range was between 0.9169 hrs (55 min) to 2.5 hrs (150 min) with a mean of 1.63 hrs (98 min). The above observations conclude that duration of analgesia and patient satisfaction is more in parturients receiving 0.125% bupivacaine hydrochloride with 25 µgms fentanyl citrate than with 0.125% bupivacaine hydrochloride alone. CONCLUSION The parturients who received a combination of 0.125% bupivacaine hydrochloride plus 25 µgms fentanyl citrate (Group-II) experienced improvement in maternal satisfaction, duration of analgesia when compared to parturients who received 0.125% bupivacaine hydrochloride alone (Group-I) given epidurally for labour analgesia.
