Comparative Study Of Complications In Posteromedial And Posterolateral Displacement In Supracondylar Humerus Fracture In Children

Abstract
The supracondylar fracture of the humerus in children is a common childhood injury accounting for 3–7% of all fractures. The Gartland type III supracondylar fracture is associated with high rate of neurovascular complications. Therefore the aim of the present prospective study was to comparatively evaluate the complications in posteromedial and posterolateral displacement in supracondylar humerus fracture. 40 patients of the age group of 4-14 years were examined for complications like radial and nerve injury or palsy as well as vascular injury at the time of presentation and were categorized into two groups (posteromedially and posterolaterally displaced fracture). All the cases were operated by open/closed reduction and cross K wire fixation. Later functional outcome was assessed using Flynn’s criteria in both the groups. The values between the two groups were compared and analysed statistically with Student t test. High rates of neural (5%) and vascular (7.5%) complications were observed in posteromedially displaced fractures as compared to posterolaterally displaced fractures which was found to be statistically significant. On the other hand posterolaterally displaced fractures were associated with better functional outcome as per Flynn’s criteria.
