Comparative Study of Intrathecal Bupivacaine with Bupivacaine Plus Clonidine in Lower Limb Surgeries

Abstract

Introduction: Initially adjuvants like clonidine was added to find out whether the addition of clonidine to bupivacaine changes its efficacy and to find out any change in quality of sensory and motor block and also to find out whether the solution was more predictable for block. We investigated and compare the characteristics of spinal block, duration of postoperative analgesia, hemodynamic responses and side effects using intrathecal bupivacaine and its combination with clonidine in patients undergoing lower limb surgeries. Material and Methods: In a present study, 60 patients of ASA grade I and II, age between 20-60 years were randomly allocated in two groups. Group A received 13.5 mg of hyperbaric 0.5% bupivacaine with 0.5 cc of normal saline and group B received 13.5 mg of hyperbaric 0.5% bupivacaine plus 75μg of clonidine. Blocks were performed after induction of anaesthesia. We studied various parameter of intrathecal block. Result: Administration of intrathecal clonidine to 0.5% hyperbaric bupivacaine significantly prolonged the duration of motor blockade, time for two segment regression and also prolonged the duration of postoperative analgesia with minimal acceptable side effects as compare to bupivacaine alone. There was no significant change in the cardiovascular response to subarachnoid block. Conclusion: Intrathecal addition of clonidine significantly prolongs the duration of motor block providing good postoperative analgesia as well as improves the quality of block.
