COMPARISON OF CONVENTIONAL PHACOFRAGMENTATION VERSUS PHACOFRAGMENTATION IN A DIFFERENT WAY FOR DROPPED HARD NUCLEUS

Abstract

BACKGROUND For dropped nucleus in the vitreous cavity, pars plana vitrectomy with phacofragmentation is the surgical procedure of choice. During phacoemulsification, we require a second instrument like chopper or Sinskey hook for chopping, but during phacofragmentation, usually, the surgeon does it with single hand, as with other hand, he holds the light pipe. Light pipe helps to support the nucleus only and helps little in chopping. This technique (conventional technique) is suitable for soft nucleus, but difficulty occurs in case of hard nucleus (black or brown cataract). MATERIALS AND METHODS Patients with dropped black or brown cataract were included. 32 eyes of 32 patients were divided into two groups- Group 1 patients undergone conventional method of phacofragmentation for dropped nucleus and group 2 patients had undergone phacofragmentation using chandelier system of light (making a fourth port) and a Sinskey hook (used as an second instrument for chopping). RESULTS The parameters like Best-Corrected Visual Acuity (BCVA), corneal oedema, uveitis, intraocular lens stability, retinal condition and macular oedema were observed and compared after two procedures and showed better results with the technique using Sinskey hook and Chandelier light system. CONCLUSION Using Sinskey hook and Chandelier system of light maybe a better technique for phacofragmentation of hard nucleus. Our data is very small for comparison. Large data is required to establish the same.
