Comparison of four different surgical techniques in cases with pilonidal sinus

Abstract
Objective: The aim of this study is to compare the results of four different surgical techniques used for pilonidal sinus. Methods: A total of 339 patients who underwent surgery for pilonidal sinus between Januray 2000 and April 2008 were evaluated prospectively. The patients were operated with one of four surgical techniques: excision and primary closure (n=76), excision and marcupialisation (n=112), excision without closure (n=104) and excision and Limberg flap (n=47) techniques. Postoperative complications (hematoma and infection) and recurrences in four groups were compared. Results: 286 male patients and 53 female patients were included in this study. Mean age of patients was 24.2 (14-57) years. The mean follow up time was 41 months. The recurrence rates after excision and primary closure, excision and marcupialization, excision without closure and excision and Limberg flap were 18.4%, 13.4%, 9.6% and %4.3%, respectively. There were not significant differences in postoperative hematoma between four groups. The postoperative infection rate in patients treated with excision without closure was the lowest but time to return to work was highest in this group partients. Patients with infection had higher rates of recurrence than patients without infection. Conclusion: Although optimal surgical technique has not been established according to our study, Limberg flap is preferable technique as compared to other techniques because of quicker healing time and decreased rates of recurrence.
