Comparison of microbiological outcomes after using Vector ® ultrasonic system and conventional periodontal treatment of chronic periodontitis in adults. Pilot study and literature review

Abstract

Introduction. Periodontitis is an infectious disease which is caused by opportunist bacteria appearing in the mouth. The presence of bacteria in combination with host genetics and other factors could lead to the loss of the remaining tooth tissue. The gold standard procedure in periodontal diseases is mechanical removal of both soft and hard supragingival and subgingival deposits, which are the main reservoir of periopathogens. In practice, the root-planing with hand instruments is the most common method of treatment periodontitis. The alternative to standard non-surgical treatment could be a ultrasonic Vector® system from the Dűrr Dental company (Bietigheim− Bissingen). Aim of the study. Comparison of the microbiological outcomes following non-surgical periodontal therapy using the new Vector® system (Dűrr Dental, Bittigheim-Bissingen) versus hand instruments in patients with chronic periodontitis based on literature review and own pilot study. Material and methods. Twenty patients with chronic periodontitis were divided into two groups in which the Vector® system was used with one group and the other treated with the conventional mechanical treatment using Gracey curettes. Microbiological testing was performed using the test Perio-Analyse® (Pierre Fabre Oral Care) identifying nine periopathogens ie: Porphyromonas gingivalis, Prevetella intermedia, Eikenella corrodens, Campylobacter rectus, Aggregatibacter actinomycetemcomitans (formely Actinobacillus actinomycetemcomitans), Fusobacterium nucleatum, Tanerella forsythia (formely Tanerella forsythensis), Peptostreptococcus micros, and Treponema denticola. Material for microbiological examination was collected from selected periodontal pockets with drains before treatment and 12 weeks after treatment. Conclusion. In our pilot study there was no statistically significant difference between the two methods detected. This means that the reduction of microorganisms obtained during treatment with the Vector® system is the same as in case of the traditional method.
