Complications of vocal fi stulas in the patients after total laryngectomy

Abstract

Introduction: Vocal fistulas are performed in patients after total laryngectomy. The indication is to provide the possibility for basic communication. Vocal fistula may be performed at the end of laryngectomy (primary vocal fistula) or later (secondary vocal fistula). Material: In the years 2005–2011 vocal fistulas were performed in 53 patients, 42 male (81.1%) and 10 female (18.9%), in the age from 35 to 75 years (mean age 54.8). 48 vocal fistulas (96.5%) were performed at the end of laryngectomy (primary fistula), 5 (9.4%) later in the postoperative period (secondary fistula). In 7 cases (13.2%) we did not observe any vocal fistula function, and in 5 cases (9.4%) fistula was closed. 5 patients (9.4%) died due to recurrences of cancer. In 2 cases (2.8%) the prolaps of fistula was observed. In 1 case(1.9%) bronchoscopy in emergency was necessary due to aspiration of prosthesis to the respiratory tract. In 3 cases (5.7%) granulomatosis in the place of fistula was observed. In 2 patients recurrence of granuloma was observed. 1 patient (1.9%) died due to multiorgan failure as a consequence of infection and hemorrhage. We did not observe any complications in the group of patients with primary vocal fistula. Conclusions: 1. Vocal fistula provides the patients with the possibility to basic communications. 2. In the cases with indications to vocal fistulas we recommend primary vocal fistula just after laryngectomy.
