Cone beam computed tomography in diagnostics of pulp extension in the talon cusp – report of two cases

Abstract
Talon cusp occurs as a result of impairment during the morphodifferentiation of tooth germ. Its anatomy is usually complicated and, if unknown, makes treatment difficult. The tooth structure assessment on the base of conventional periapical radiograph is always insufficient. The possibility of misinterpretation in radiological diagnostics at early stage of tooth eruption is suggested. Recently the cone beam computed tomography (CBCT) for dental purposes was developed. The main advantage of CBCT diagnosing is that the patient receives significantly lower radiation dose compared to conventional computed tomography (CT). There are only 2 reports related to using of three-dimensional (3D) images in these cases in adults. The present study shows CBCT used in children in fully developed taloned teeth before caries therapy. This method appeared to be useful to determine pulp extension in the cusp and to prevent the tooth vitality. CBCT can improve diagnostics, treatment planning and it allows to be more sure in the instrumentation. A new method could facilitate diagnosis especially in immature teeth usually requiring invasive treatment during eruption.
