DEPRESSIVE FEATURES AND SUICIDAL IDEATION IN ADULTS WITH OBSESSIVE COMPULSIVE DISORDER- A HOSPITAL BASED CROSS SECTIONAL STUDY

Abstract
BACKGROUND Obsessive Compulsive Disorder (OCD) is a chronic distressing anxiety disorder associated with significant functional impairment. Because of the demoralization and hopelessness caused by obsessive compulsive symptoms, these patients view their life as worthless and develop marked suicidal ideation. We wanted to study the depressive features and suicidal ideations in Indian patients with OCD. We also wanted to study the relationship between sociodemographic data, clinical symptoms, severity of OCD and the outcome variable (depressive features and suicidal ideations). METHODS After approval from Institutional Ethics Committee and after getting informed consent from patients, as per the exclusion and inclusion criteria 60 patients (N=60) fulfilling the ICD-10 Criteria for Obsessive Compulsive Disorder (as per ICD -10) were taken up for the study through continuous sampling method. A semi-structured proforma was used to fill the socio demographic data, family history, duration, and type of disorder. Yale Brown Obsessive Compulsive symptom check list and Yale Brown Obsessive Compulsive Scale (Y-BOCS) were applied to assess symptom profile and severity of OCD. Montgomery and Asberg Depression rating scale (MADRS) was used to elicit the severity of depression. Beck's suicidal ideation scale was administered to assess the suicidal ideation. Data was analysed in MS- Excel. Chi-square test was used to find the association between categorical variables. A āpā value of <0.05 was considered statistically significant. RESULTS The baseline sample included 60 patients among which 37 (61.7%) were men and 23 (38.3%) were women. Mean age of the patients was 32.5 years. Age at onset of illness was 27.2 years. More than 95% of the patients were in the low and middle socioeconomic status. Severity of OCD symptoms was highly significant with the magical thinking and washing symptoms. Severity of OCD symptoms was directly proportional to depressive features. Suicidal ideations were directly proportional to the severity of the depressive features. CONCLUSIONS Specific OC symptoms (magical thinking and washing compulsions) have increased severity of OCD which in turn increases the severity of depressive features leading to suicidal ideation.
