ДИАГНОСТИКА КОГНИТИВНОЙ ДИСФУНКЦИИ В ПОСЛЕОПЕРАЦИОННОМ ПЕРИОДЕ

Abstract

The aim of the study is to select adequate neuroprotective therapy for patients with postoperative cognitive dysfunctions based on subtraction of the total cognitive deficiency. The study was conducted in surgical departments of various profiles on the basis of the Kharkov City Clinical Hospital for Emergency and Emergency Medical Care named after Professor A. I. Meshchaninov. The study was conducted in patients of different age groups: young, middle and elderly. The state of cognitive function in patients with acute surgical pathology was performed before surgery and on the 1st, 7th, 30th day after surgery. The study of cognitive function: scale MMSE, test drawing hours, test "10 words", battery tests for frontal dysfunction, method Schulte. The calculation of the total cognitive deficiency was carried out. In each age group, on the seventh day of the study there were patients with a restored state of cognitive function at the preoperative level. Some patients had minor changes in cognitive function. In some patients, cognitive impairment was observed in comparison with the preoperative period. The results of the change in cognitive function in different periods of the study and in different age groups made it possible to develop treatment regimens. Significant changes were noted in 28 % of patients (mean age 68.9 years) on day 7. According to MMSE - below the norm by 40 %. The test results for drawing hours are below the norm by 30 %. On the FAB scale, patients have a deficit of 22 %. By the method of A. R. Luria - lower by 60 % of the norm. According to the method of Schulte – 44 % below normal. OKD = 39.2 %. Treatment is recommended on the basis of diagnosis of the degree and structure of cognitive dysfunction for the analysis of results on the 7th day after the operation. Based on the results of the study, we developed and proposed a formula for calculating the total cognitive deficit. The data obtained by us on the cognitive function in surgical patients after surgery in the conditions of general anesthesia made it possible to formulate a formula for calculating the index of the total cognitive deficiency. This allowed to form a tactic of further correction of the existing cognitive deficit with the use of neuroprotective therapy.
