Displacement crackles in temporomandibular joints and the quality of abduction and adduction mandible track in functional tests, using Zebris JMA device

Abstract

Aim of the study. To evaluate the impact of displacement crackles, located in the temporomandibular joints, on the quality of graphic records and free jaw movements, recorded using a Zebris JMA device and to determine the usefulness of instrumental techniques in the analysis of functional diagnosis of disorders in the stomatognathic system. Material and methods. Fifty five people (36 women and 19 men, aged 15–66 years) were eligible for the study. They were divided into three groups: group I included those aged > 25 years (19 individuals); group II those aged 25-35 years (16 individuals) and group III those aged <35 years. Using the RDC / TMD criteria they were also divided into two subgroups: those without acoustic symptoms in the TMJ and with articular disc displacement without its bloking – IIA. The second stage of the study was carried out using a Zebris JMA device to define functional status of the stomatognathic system – jaw locomotion (type of deviation) in the movements of abduction and adduction.Results. Acoustic noises of the temporomandibular joint - displacement crackles, audible in the stethoscope study, were recorded in a total of 41 examined, and also in 10 people who were not qualified for group IIA according to the BKD / ZCURNŻ criteria. A statistically significant relationship between the occurrence of acoustic noises in TMJs and the gender was revealed. The presence of displacement crackles in both joints determines in 94.74% the deviation of the lower jaw in the form of S-shaped track. Conclusions. Acoustic symptoms – displacement crackles are the major determinant of qualitative changes in the graphic records of tracks abduction and adduction of the mandible. The survey of instrumental technique using a Zebris JMA device makes it possible to capture even unobtrusive changes in the functioning of the TMJ, which may be unrecognizable during a traditional, clinical trial.
