Does Rifampin and Ceftriaxone Can be an Alternative to Vancomycin and Ceftriaxone in Experimental Penicillin and Cephalosporin Resistant - Pneumococcal Meningitis ?

Abstract

Objectives; The antimicrobial therapy of meningitis caused by Streptococcus pneumoniae resistant to penicillin and cephalosporin has been changed. Although the combination of ceftriaxone and vancomycin has been accepted as the most appropriate choice for pneumococcal meningitis therapy resistant to penicillin and cephalosporin many antimicrobial agents have been under investigation. The present study was planned to compare the efficacy of ceftriaxone and vancomycin combination with ceftriaxone and rifampin combination in rabbit model of meningitis caused by penicillin and cephalosporin resistant strain of S.pneumoniae. Methods; Thirty six rabbits were grouped into three. Meningitis was induced in all rabbits by intracisternal inoculation of S.pneumoniae strain, resistant to penicillin (MIC 2 µg/ml) and extended spectrum cephalosporins (MIC 0.5 µg/ml). Group 1 was given placebo while group 2 and 3 were given ceftriaxone and vancomycin, and ceftriaxone and rifampin respectively. Two doses of vancomycin and rifampin 12 h apart were given by the marginal vein of the rabbit ear while ceftriaxone was given in single dose. Cerebrospinal fluid bacterial concentrations were mesured at 0, 2, 12, 14, and 24h after therapy was started. Results; In the control group bacterial growth was seen at 0, 2nd, 12th, 14th, and 24th h. At 2nd h the mean cerebrospinal fluid bacterial concentrations were 2x106 and 3x106 in group 2 and 3 respectively. But no growth was observed in both the ceftriaxone and vancomycin, and ceftriaxone and rifampin groups at 12, 14, and 24h. Conclusion; It is concluded that ceftriaxone and rifampin combination was found to be as effective as ceftriaxone and vancomycin combination. Rifampin can be the alternative choice of vancomycin especially when the proinflamatory cytokin levels are considered to be an important prognostic factor and whenever dexamethasone therapy is necessary in the treatment of meningitis caused by S. pneumoniae strain.
