Dysfunctions of the masticatory apparatus in patients after mandibular reconstruction

Abstract

[b] Introduction.[/b] Since 2010, about 4,000 oral cavity cancer cases and about 2,500 deaths have been reported in Poland. The five-year survival of patients with oral cancer is less than 50%, which is associated with a low level of health education of the population and problems with the implementation of prevention programs. Surgical treatment of oral cancer involves surgical resection of the tumor with a safe tissue margin and reconstruction of the resulting loss with an autograft. Surgery is often associated with chemotherapy and/or radiotherapy. One of the consequences of surgical treatment of malignant cancers of the oral cavity are dysfunctions of the temporomandibular joints manifested by problems with biting off, chewing, forming food boluses and swallowing, which negatively affect daily functioning and the quality of patients’ lives. Dysfunctions of the entire orofacial complex, especially those of the temporomandibular joints are an effect of reconstructive surgery of the oral cavity. Such disturbances encompass the occurrence of pain, and chewing, swallowing and speech problems. [b]Conclusions.[/b] Surgical resection and reconstruction involving the mandible adversely affect the functioning of the temporomandibular joints. The resulting dysfunctions require therapeutic action and cooperation with an interdisciplinary team of specialists in dentistry and physiotherapy. The aim of this study is to present everyday problems of patients after surgical reconstruction of the mandible and to highlight their importance in the treatment process conducted by specialists in dentistry, orthodontics, prosthetics and physiotherapy.
