EFFECT OF DIFFERENT POSITIONS ON VARIOUS VITAL PARAMETERS OF NEWBORNS ADMITTED IN NICU

Abstract

Aim: To study the effect of different positions on various vital parameters of newborns admitted in NICU. Methodology: This prospective observational study conducted in the department of paediatrics at Shri Guru Ram Rai Institute of Medical and Health Sciences (SGRRIM & HS), Dehradun, Uttarakhand during the period of one year. A total of 300 neonates admitted in N.I.C.U. were placed in 8 positions, namely prone 0°(P0), prone 30°(P30), supine 0°(S0), supine 30°(S30), left lateral 0°(LL0), left lateral 30°(LL30), right lateral 0°(RL0) and right lateral 30°(RL30). Oxygen saturation, heart rate and respiratory rate were monitored and recorded after 30 minutes in all 8 positions. The data obtained was subjected to statistical analysis (SPSS version '17') using paired t test and anova test for various intragroup and intergroup comparisons respectively. Results: The mean oxygen saturation was 96.16±1.53, 96.16±1.6, 95.02±1.72, 94.33±1.98, 97.22±1.56, 96.52±1.53, 94.72±1.84 and 94.48±2.07 in P0, P30, S0, S30, LL0, LL30, RL0 and RL30 respectively. Similarly the mean heart rate was 143.36±7.47, 143.29±7.22, 146.95±7.12, 149.89±7.34, 140.25±7.43, 142.08±7.33, 148.43±7.06 and 150.27±7.44 in P0, P30, S0, S30, LL0, LL30, RL0 and RL30 respectively. The mean respiratory rate was 53.03±7.04, 53.35±7.04, 56.20±7.02, 58.552±7.19, 50.53±6.75, 52.27±6.91, 57.33±7.11 and 58.27±7.27 in P0, P30, S0, S30, LL0, LL30, RL0 and RL30 respectively. The difference in the vitals was statistically signi􀃶cant (P<0.001) among four positions with 0 and 30 degree elevation. But the difference in the vitals was not statistically signi􀃶cant between right lateral 0 and right lateral 30 degree. Conclusion: The prone 30 degree position favoured better vital parameters than other positions.
