Effectiveness Of Platelet Rich Plasma (Prp) Injections In Management Of Chronic Plantar Fasciitis: A Randomized Controlled Trial

Abstract

Background: Plantar fasciitis is the most common cause of heel pain. With early diagnosis and timely application of conservative methods, symptoms generally resolve over time. However, despite adequate treatment, 20-30% of patients will experience persistent symptoms and develops chronic planter fasciitis. In these patients, local platelet rich plasma injections initiate a regenerative tissue-healing cascade which has greatest potential to resolve long-standing symptoms. This study was designed to evaluate its effectiveness by comparing the PRP injection and conservative method with conservative method alone. Materials and Methods: The study was carried out among 179 patients with a clinical diagnosis of plantar fasciitis, divided randomly to case (n = 91) and control (n = 88) groups. The case group received PRP injection and conservative method. The control group received conservative method alone. The protocol was the same in both group. We used visual analog score [VAS] for the assessment of pain relief in both groups. Results: After along fallow up , the mean VAS had decreased significant in the case group from 6.59+-1.07 before treatment to .54+-.60 at last fallow up 52 week (P< 0.05). After 12 weeks there was gradual increase in mean VAS from 5.9 at 12 weeks to 6.2 at 52 week. In control groups the mean VAS at the start of study was 6.5±.99 and at 12 weeks we able to achieve VAS up to 5.9±’86 only with conservative management without any PRP injection. After 12 weeks there was gradual increase in mean VAS from 5.9 at 12 weeks to 6.2 at 52 weeks. When both treatment methods were compared then it was observed that these two methods differ significantly from immediate to 52 week (p≤.05). Conclusion In this single-centre randomized case control prospective preliminary study, results indicate that on long term follow up PRP injection have the potential to reduce pain than conservative methods alone in chronic cases of planter fasciitis.
