Effects of ultrasound guided transverse abdominis plane block on postoperative analgesia in patients undergoing unilateral inguinal hernia repair

Abstract
Ultrasound guided tap block has been described with promises of better localization and deposition of the local anaesthetic into the plane with improved accuracy. The aim of this study was to evaluate whether tap block reduces 24 hour diclofenac consumption, prolongs the duration of analgesia and delays the time to first request of diclofenac when used after inguinal hernia surgery. Background : After approval of ethical committee, 60 patients scheduled for elective hernia repair were included in this study. Group 1 (n=30) received tap block with 0.5% ropivacaine 20 ml at the end of surgical procedure and Group 2 (n=30) were taken as control. All patients were premedicated with midazolam 0.02 mg/kg intravenous and received 3 ml heavy bupivacaine with 25 μg of fentanyl. Vitals and VAS score for pain were assessed at regular intervals postoperatively. 24 hour diclofenac consumption, rescue analgesic requirement, time to first demand of analgesic and complications related to tap block were also noted. Methods : Postoperatively pain scores at 10 minutes interval was comparable between both the groups and at rest of the time intervals it was found to be highly significant in Group 1. Also in group 1 we noted reduced demand of analgesics (P <0.001) and rescue analgesics (P <0.01). The mean time to first request of analgesic in group 1 was 267.27±172.05 min and 149±48.87 min in group 2 (P<0.001). Results : USG guided tap block is simple, safe and provides effective postoperative analgesia in patients of unilateral inguinal hernia repair.
