Emergency Referred Cases in Obstetrics: An observational study at Tertiary Care Teaching Hospital

Abstract
Background : Worldwide every year approximately 8 million women suffer from pregnancy related complications. Majority of maternal complications are avoidable when women have access to healthcare before, during and after childbirth. It is now universally recognized that the most effective strategy for reduction in foeto-maternal morbidity and mortality due to complications of pregnancy and delivery is to provide emergency obstetric care services within reach of all pregnant women. To evaluate the reasons for emergency reference in obstetrics and foeto-maternal outcome in this cases. Aims and objective: This is a prospective study of 100 emergency referred cases at our tertiary care teaching hospital during period of June 2015 to May 2017.Evaluation of patients by detailed history, clinical examination, routine and special investigations and ultrasonography were carried out. All the Post-partum patients were excluded. Material and method: In present study majority of patients were primiparous(53%) with age group 21-30 years(85%)and having less than 3 antenatal visits(61%). Most common cause of referral is anemia (17%) followed by Previous caesarean section (9%) and PIH(12%).Most common complication is postpartum haemorrhage. Maternal mortality in 2%cases and stillbirth in 6%. Results: It is found that poor socio-economic status, lack of human resources and facilities at many subcentre are the pitfalls in implementation and utilization of our community obstetric care services. Foeto-maternal morbidity and mortality can be decrease by early diagnosis and timely reference and intervention during antepartum or intrapartum period.
