Endoscopic Tympanoplasty Type I: Impoving Techniques

Abstract
BACKGROUND AND OBJECTIVES: Transcanal endoscopic tympanoplasty type I [TET 1] is a very advantageous procedure and has got universal acceptance now emerging from the resistance that it faced about two decades ago. This minimal invasive surgical technique has reduced the duration of hospital stay, post-operative pain and discomfort, and has also improved the percentage of uptake of graft and closure of air-bone gap in the post-operative pure tone audiogram. A constant endeavour is to improve its results by improving the techniques. Our study is about the evolution in the technique of the endoscopic tympanoplasty type I in an otolaryngological surgical unit, in the quest for improved result in treating tympanic membranes perforations in Chronic Supurative Otitis Media [CSOM]. METHODS: A study of TET 1 performed by a single surgeon in the last four years have been recorded. Comparison was done between classic technique of putting absorbable gelatine gel in middle ear and avoidance of the same. RESULTS: Keeping the middle ear free from any foreign body including absorbable gelatine gel improved the post-operative result. The technique of TET 1 achieved all-round improvement in post-operative reduction of pain, weakness, dizziness, duration of hospital stay to less than 6 hours, increased percentage of graft uptake to nearly 100 %, air-bone gap to near normal hearing and excellent cosmetic results. CONCLUSIONS: TET 1 is a minimal invasive surgical technique to repair tympanic membrane perforations caused by CSOM. It is going to be the future of hearing improvement surgery.
