Enterotoxin gene profiles of Staphylococcus aureus isolated from environment and patients in a hospital in Shanghai from 2018 to 2021

Abstract
ObjectiveTo investigate the prevalence of Staphylococcus aureus in a hospital in Jiading Districtï¼ Shanghaiï¼ and to determine the enterotoxin gene profiles of Staphylococcus aureus isolated from patients and environment.MethodsSpecimens were collected from environment and patients from a hospital for Staphylococcus aureus isolation and identification. Furthermoreï¼ enterotoxin genes ï¼SEAâSEEï¼ of Staphylococcus aureus were detected.ResultsA total of 54 Staphylococcus aureus strains were isolated from 780 hospital environmental specimens from 2018 to 2021ï¼ with a prevalence of 6.92%. In the armrests in the wardsï¼ patientâs pillows/quiltsï¼ and bedside cupboardsï¼ the prevalence was determined to be 20.00%ï¼ 20.00% and 16.67%ï¼ respectively. In contrastï¼ in the computerâs mouse and keyboard and work clothes of physicians and nursesï¼ the prevalence was 17.42% and 16.67%ï¼ respectively. Meanwhileï¼ from 2018 to 2021ï¼ a total of 75 strains were collected from patientsï¼ of which 36.00%ï¼ 14.67% and 14.67% were from the departments of intensive care medicine ï¼ICUï¼ï¼ neurosurgery and orthopedicsï¼ respectively. The prevalence of Staphylococcus aureus enterotoxin genes were 48.15% and 61.33% in the environment and patientsï¼ respectively. Furthermoreï¼ the prevalence of SEAï¼ SEBï¼ SECï¼ SEDï¼ and SEE genes were 14.67%ï¼ 41.33%ï¼ 9.33%ï¼ 1.33%ï¼ 1.33% in patientsï¼ respectively ï¼ whereas 20.37%ï¼ 25.93%ï¼ 1.85 %ï¼ 1.85% and 0% in environmental specimensï¼ respectively. In the environmental specimens isolated from comprehensive ICUï¼ the prevalence of enterotoxin genes was 77.27%. In the patient's specimensï¼ Staphylococcus aureus was mostly isolated in sputum. Additionallyï¼ the prevalence of enterotoxin genes was high in the patients of departments of respiratory medicineï¼ ICUï¼ and orthopedics.ConclusionPrevalence of Staphylococcus aureus remains moderate in the hospital environment. Enterotoxin genes of Staphylococcus aureus are commonï¼ with seb gene as the most common geneï¼ followed by SEA gene. It warrants strengthening the disinfection and control of Staphylococcus aureus in the hospitalï¼ especially in the ICU.
