Eruption disturbances of first and second permanentmolars: classification, etiology, differential diagnosis and treatment

Abstract

Eruption is a complicated process involving the axialmovement of the tooth from its place of development tothe functional position in the occlusal plane. This processcan be affected by many factors. Retained or delayederuption of the molars, due to different etiology, clinicaland radiological symptoms and method of treatment isdivided into ectopic eruption, retention/impaction, primaryretention, and secondary retention. Ectopic eruption ischaracterized by the mesial deviation of the eruption trackand the adjacent primary/permanent tooth resorption.It can be a temporary eruption disorder or it can lead to tooth impaction. Tooth retention caused by a physicalobstacle in the tooth eruption path is treatable by surgicalexposure and orthodontic extrusion of the tooth. Primarytooth retention occurs as a result of a genetic disorder ofthe eruption mechanism (PTHR1 gene mutation).A characteristic feature of this anomaly is that it inflictsthe teeth distal to the first affected tooth. Since ankylosisoccurs so frequently as a secondary effect that accompaniesthe operation of orthodontic forces, extraction andprosthetic treatment are the only solutions. Ankylosisrepresents the primary cause of secondary tooth retention.Most commonly, it affects a single tooth in which advancinginfraocclusion leads in extreme cases to a repeatedembedding of the tooth in the surrounding tissues.Although in some cases surgical tooth luxation allows fora minor orthodontic extrusion, there is usually no responseto orthodontic treatment and tooth extraction is necessary.The knowledge of different types of tooth retention enablesthe clinician to arrive at an adequate and effectivetreatment decision, and such treatment can protect thepatient from dangerous health complications.
