EVALUATION OF CUMULATIVE ANTIMICROBIAL SUSCEPTIBILITY DATA OF ESCHERICHIA COLI, KLEBSIELLA PNEUMONIAE AND STAPHYLOCOCCUS AUREUS BACTERIA ISOLATED FROM BLOOD CULTURE

Abstract
OBJECTIVE: In this study, cumulative antimicrobial susceptibility data of Escherichia coli (E. coli), Klebsiella pneumoniae (K. pneumoniae), Staphylococcus aureus (S. aureus) bacteria isolated from blood cultures were presented and it was aimed to contribute to antimicrobial selection in the treatment process. MATERIAL AND METHODS: Blood cultures studied in our laboratory between January 2021 and December 2021 were included in the study. The first isolate for each patient was included in the study. Cumulative antimicrobial susceptibility reports were obtained by retrospectively examining the antimicrobial susceptibility test results of S. aureus, E. coli, K. pneumoniae isolated from blood cultures. RESULTS: In our study, methicillin resistance in S. aureus was found to be 31.4%. Extended-spectrum beta-lactamase (ESBL) rate was determined as 68% in E. coli isolates and 88% in K. pneumoniae isolates. The susceptibility of E. coli isolates to gentamicin, amikacin, and piperacillin+tazobactam was 46%, 66%, and 54%, respectively, and the susceptibility of K. pneumoniae isolates to amikacin, gentamicin, piperacillin+tazobactam, and ciprofloxacin was 30%, 21%, 13%, and 10%. CONCLUSIONS: The most appropriate antibiotics for empirical treatment of S. aureus isolates were vancomycin, teicoplanin and linezolid. In addition to these antibiotics, gentamicin appeared to be an option. In E. coli isolates producing ESBL, only carbapenem antibiotics were found suitable for empirical treatment. Carbapenem resistance was also found to be high for ESBL positive K. pneumoniae. Due to the high resistance rates obtained in our study, regular surveillance is required. Further studies are needed to test the efficacy of antibiotic combinations for multi-resistant microorganisms, including carbapenems.
