EVALUATION OF PERCEPTION IN PATIENTS WITH CARPAL TUNNEL SYNDROME USING VON FREY’S FILAMENTS

Abstract

Introduction. Median nerve compression syndrome at wrist is diagnosed using standard clinical trials, but selected clinical neurophysiology studies (especially electroneurography transmission studies in sensory and motor fibers of the median nerve as well as needle electromyography in abductor pollicis brevis muscle) can be the valuable supplementary to the confirmation of this pathology. Non-invasive, non-standards testing like surface electromyography (sEMG) and surface sensory perception studies are used especially for assessing the progress of treatment, either conservative or surgical. Aim and methods. This study examined the validity of clinical neurophysiology studies like sEMG and surface sensory perception with von Frey’s filaments (FvF) for the diagnosis and evaluation of treatment in 5 patients after rehabilitation and 7 patients after surgery with carpal tunnel syndrome confirmed in studies of ENG (electroneurography) applied to the median nerves. Results. In the light of this work, the decisive importance in the diagnosis of carpal tunnel syndrome, have especially conduction studies of fibers, not only the motor in medians nerves, but ulnar as well. These diagnostic methods allow, depending on the stage of carpal tunnel syndrome advancement, on the choice of conservative treatment or surgery. Changes in the superficial perception of sensory innervation with the reference to median and ulnar nerves neuropathy are the sensitive indicator for their neural transmission assessment. Result of surface EMG recordings suggested improvement in activity of motor units in examined hand muscles following applications of applied therapy. Conclusions. Based on the results of researches conducted in this study, impaired sensory perception in the case of the median nerve neuropathy at the wrist does not precede abnormalities in the transmission of motor fibers.
