Evaluation Of Various Surgical Modalities For Treatment Of Fracture Distal End Radius

Abstract

Background: A prospective study was conducted in the department of Orthopaedic & Traumatology in M.Y Hospital, Indore from November 2013 to October 2015. Methods: All the patients with fracture of distal end radius with or without ulna fracture were admitted in our hospital over a period of 2 years as per our inclusion criteria. A total of 55 patients were included in our study and various surgical modalities were assessed among the various available treatment modality such as percutaneous fixation with k-wire, external fixation & plate osteosynthesis using Volar locking plate. Fractures were classified according to the AO classification into type A (extra-articular), type B (partial articular) and type C (complete articular) and dividing them into three pools as per fracture type. Observations & Results: Out of the total number of cases (n=55) for fracture distal end radius, 36.63% cases were managed percutaneous fixation with k-wire (n=20), 36.63% with external fixator via ligamentotaxis with or without k wire (n=20), 27.27% with plate osteosynthesis using volar locking plate (n=15). Anatomical results were evaluated according to the Sarmiento’s modification of Lindstrom Criteria. Clinical and functional results were evaluated according to the demerit point system of Gartland and Werley with Sarmiento modification. Conclusion: There is no unambiguous modality treatment for all the fractures of the distal end radius. The treatment choice should be based on the fracture type and geometry, degree of comminution, extent of soft tissue involved ,the patient’s profile (age, osteoporosis, systemic illness), the patient’s demands and last but not least on the treating surgeon’s expertise and preference. As long as stable reduction and anatomic reconstruction are guaranteed by appropriate modality as per above confounding factors, many ways can lead to a successful treatment.
