Family nurse’s practices while providing medical care to the patient with chronic cardiovascular disease

Abstract

Background: Cardiovascular diseases belong to the group of the most common chronic diseases in Poland and in the whole world. Despite the fact that there are prophylactic actions taken, cardiovascular diseases are still the main cause of death. Family nurses play a crucial role in the non-hospital healthcare. They are a part of a basic healthcare therapeutic group providing an extensive care to the patient. Within the responsibilities of family nurses, we may specify: self-diagnosis, planning and implementation of nursing care. The family nurses are also a source of information for a patient; they implement a care and also therapeutic, diagnostic or rehabilitative actions. These actions are taken according to the initially settled model of healthcare, binding norms, the Code of Professional Ethics or legal acts regulating nurses’ practices.The aim of the study: The aim of the work is a presentation of family nurses’ actions taken while nursing a patient with chronic cardiovascular disease. The presentation is based on a case of a patient with arterial hypertension.Material and methods. The material is a description of the health situation of the patient, who reported to the family nurse diagnosed with hypertension. It was constructed on the basis of individual case method, which uses two techniques: analysis of documentation and interview.The case description: In the following work there is presented a case of nursing a 45-year-old patient with diagnosed hypertension. Lack of any other chronic diseases. Patient with family burden; nicotine addiction for 20 years revealed in the interview. Antihypertensive medications are taken irregularly. The main nursing interventions were directed to prepare the patient to an active participation in the process of self-care.Conclusions: Nurses, who are a part of therapeutic team of basic healthcare, deal with a range of exercises connected with nursing a patient with chronic cardiovascular disease. Apt and precisely defined nursing diagnoses determine the following interventions directed towards a proper care over a patient. Education and conservative approach guidance in the course of arterial hypertension allow a proper preparation of the patient to a self-care.
